You need to fill out the form because there is a penalty if you do not. Individual is the correct box if you are a human being. The only situation in which that box should not be checked is if the mortgage is being paid by a corporation or partnership. The reason that they keep having you do this form is because the computer thinks that the SSN and name do not match. It could be a spelling mistake.
