What exact forms do I need to provide the auto loan lender for banktuptcy?

I have an auto loan that the lendor wants to reposess, but Im filing bankruptcy and want to re-affirm the debt with them because its the only debt I want to keep because I want to keep the vehicle. I also have 6 other credit cards I want to file with. What exact forms do I need to provide the auto loan lender? Im doing the bk myself. What do I do when I call them? Do I tell them I'm filing? I'm in NH if that helps at all.
