You have to call the card company. They will probably want you to fill out a police report as well. There will possibly be a criminal investigation as well. If you are willing to go that far you will probably get a refund.

Depends on how far you want to take it. To stop further charges, stop your credit card with you provider first. If you can't get your money back from your cousin, then it is up to you whether you report it to the police. But keep in mind that it could get ugly.

If you allowed your cousin to use your card (even if it wasn't for those items in particular) you don't have any recourse. Credit card companies will only usually give you credit if your card was stolen. And if it was stolen, you need to have reported it stolen, and given your personal relationship with the thief, you probably will need to file a police report. Talk about awkward family reunions- good luck! :)

If you authorized ANY charges by this cousin, then you are entitled to a refund of ANY charges by this cousin. If you authorize ANY person to use your card, then you are responsible for ALL charges by that person. If you did not authorize your cousin to use this card at all and this cousin stole your card, then you are entitled to a refund. However, reporting it could result in your cousin being arrested. Taking that risk for only $400 may not be a good decision.
