Yes, it has to be listed that way in the MLS and fully disclosed to any buyers.

yes, the real estate listing must state this in clear terms. it will often be in bold right at the top of the listing.

No No. Not necessarily. First of all if a seller has cash to bring to closing then no lender approval is needed. In these cases the listing does not need to make any reference to short sale. If lender approval is required then yes listing should include a reference. Note it may not be visible to the public, but only those who have access to mls. It is not required but certainly good practice.
