First, check with your Secretary of State office by email to see if the name you have in mind is available. Also review the rules for corp. names to be sure the name complies. Then you would file Art of Inc (It is Form 202 now with a new name) for a corporation. After you become a Corporation, THEN you can elect with the IRS that you wish to be considered an S-Corp. Get EIN # from IRS (Easy by website or phone) Easiest way to accomplish this is go to LEGALZOOM.com. It is very neat and orderly and you can also get the corporate binder, seal, and invaluable forms and templates, checklists. WORTH IT Legal Zoom walks you through the whole process.
