Why would my insurance company deny to pay for my CT Scam?

I lost my Peripheral vision followed by a intense headache. Went to the doctor the following monday and the Nurse Practitioner ordered a head CT after 2 cancelled appointment because they didn't get back with the pre autherization in time now they say it's denied. Why?

it was not authorized so it is not covered.

They should be telling you, in writing, whey they are denying coverage for this.
