What do I need to fill out on the W-9 form that my mortgage company sent me?
My name is already printed and so is my SS#. I know I need to sign and date. My questions is in the section that states "Check appropriate box: Individual/sole proprietor" etc. I checked that box but now I am not sure that I should have done that. Anyway, since my information has not changed since I filled this out last year, why does the IRS keep having me fill out this form? It states it sent because the name and TIN combination for the account listed is inaccurate or incomplete. Untrue.

you get this because the SS number and your name do not match what you file your income taxes under.
it came from the mortgage company, but got generated because when they reported your interest paid to the IRS, the IRS said "name and SS number do not match -- resubmit".
it is perfectly possible to own property in a name other than the one on your tax return, and to borrow money under that name.
The problem should be sorted if you send the IRS a change of name form [recent divorce? spouse died and you took your name back?], or if you can get the mortgage company to use your IRS name in the IRS filing.

Give form to the requester. Do not send to the IRS. Form W-9 Request for Taxpayer Identification Number and Certification
The form does not come from the IRS look at the return address for the form and contact them and them why they keep requesting this information from you and make sure that your name on your social security card matches the social security number that is on this W-9 before you sign it and return it to the sender (requester)
Some thing must be wrong with the information so the requester may be able to explain to you what is wrong.

You need to fill out the form because there is a penalty if you do not.
Individual is the correct box if you are a human being. The only situation in which that box should not be checked is if the mortgage is being paid by a corporation or partnership.
The reason that they keep having you do this form is because the computer thinks that the SSN and name do not match. It could be a spelling mistake.
