No, Not all buisinesses start out large:
Pixar,
Microsoft,
Boscovs,
Hollywood,

All examples of buisinesses that started out with over 20k employees world-wide. they were financed by entrapeneurs who either knew from the getgo what they were doing, or who struck lucky without having to go through the process. I would say 9.5/10 buisinesses started out small though. You have to start somewhere.

Yes, it is. For example, about 80% of existing Target stores started construction and opened on the same exact days across the nation.

Most successful businesses started small because they want to test their products and services for their target market. When you start small, you can be hands-on with your business and you gain experience on how to manage it well. The reason why some experts suggest starting small business first is because it will help the enterpreneur to get familiar with the business. If he fails, it won't hit him that hard because he invested only a small amount of capital.

Starting large scale is good for trained and experienced businessman because they know how to handle the business and they have the resources, networks and people to help them run the business.

I also included here a link on how to start small business.

No, not all businesses start small.

Pixar films is a great example:
Pixar began as a division of LucasFilms called The Graphics Group. This is the LucasFilms as in George Lucas and StarWars fame.
It was later "spun out" into into it's own company, completely separate from LucasFilms and renamed - Pixar.

If something is built from scratch - it always starts small, but it can grow very quickly:
Pinterest was started by 3 guys (not girls... isn't that funny - almost their whole market is girls) in December 2009. The official website launch was in March 2010.
- I read today that Pinterest is worth $2.6Billion.
