I think you will not get the exact answer to your question over here, try to google some seo companies online and try to contact them. You will get different packages from the different companies. Just need to do a little bargaining with them if you can.

I think $100/month is a good budget for 5 keywords. It could take 6-12 months to rank top. There are 2 types of SEO provider,
