Question about mailing something...?

Ok this might sound stupid but I'm using someone as a reference for a project I have coming up. The director gave us envelopes with the return address already printed on there, which is the school's address. Then he put as the mailing address his name with the school's address again as the mailing address. So my reference tried mailing it over a month ago and it never showed up.

Did it not show up because the return address and mailing addresses are the same? Can they mail something this way? Please help I need this issue resolved.

I'm guessing your letter just got lost. Back in my songwriting days, many times I have done what's called a poor mans copywrite, and mailed a song I had written to myself, and left it sealed should I ever need to prove the date I wrote it. I always received them without a hitch, and yes the main address and return address were always the same.
